Measuring lateral margins in vulvar squamous cell carcinoma


Introduction

We’re conducting research on measuring lateral margins in vulvar squamous cell carcinoma (VSCC). And therefore
we are interested to study how pathologists measure these margins. This online survey might contribute to a practical
guidance to improve existing guidelines.This survey will take ~10 minutes of your time.

Rationale

VSCC are notorious for their high recurrence rates considering that approximately 25% of patients develop a
local recurrence after primary surgery. Most guidelines recommend a minimal tumour-free margin of 8mm which
is based on two retrospective studies in 1990 and 2002.
Many studies which questioned whether pathological margin distance matters in recurrence rates show discrepant
results. This might be explained by the different definitions in local recurrences, but also by the lack of a
consensus guideline on how to measure margins in VSCC.

Goal

We propose this practical guideline to standardize the margin distance. We selected usual cases of VSCC, and
send these out as an online survey to all attendants of the British Association Gynaecological Pathologists
(BAGP) meeting. You will find the general instructions below.
The results will be presented at the BAGP meeting 2018.

General instruction

  • Please open the online survey one time, and finish in the same session.
  • Only measure lateral margins, not the deep margins.
  • Only measure the smallest distances when a case with two margins is presented.
  • Not all cases are presented with an ink mark. We strive to present cases that pathologists encounter in daily practice.

How to start

  1. Open the survey in Google Chrome or Safari.
  2. Click ‘introduction’.
  3. Answer the questions on the left side (4 multiple choise questions).
  4. Click ‘submit’.
  5. Click case 1.
  6. Click ‘measure’ button in green.
  7. On the left upper corner of the H&E photograph tools will appear.
  8. When you want to draw a new line for measuring, you can just start a new line. The old line will disappear.
  9. If you want to zoom in, and move the image you can click the button with two arrows.
  10. When you are satisfied with your measurement, click ‘submit and new’.
  11. The next case will be presented (repeat step 6-9).
  12. When all cases in the ‘case list’ are measured, you can close the survey.
